COVID-19 Visitor Health Check Form

Beaver County is committed to the health and safety of our community and staff.

Effective August 12, 2020, the County Services Centre will re-open to the public with access restrictions.  The Services Centre main door will not be locked, however only 2 (TWO) visitors will be allowed in the lobby at one time.

To avoid line-ups, the County strongly encourages residents and visitors to take advantage of the main options for conducting business.

When attending the County Office on routine business (e.g. paying a bill), a short mandatory health check is required.  You may also consent to sign the optional contact tracing register.   Masks are required and one will be provided to you if you did not bring a mask with you.

Appointments with staff should only be made when other forms of communication cannot resolve an issue.  If an in-person meeting is necessary, in addition to the short mandatory health check, a comprehensive health check will be required.  Please come prepared by downloading, printing, and complete the COVID-19 Visitor Health Check Form within 24 hours of your visit.
